Lead Capture Forms
Collect contact info before a conversation starts. Useful when an AVCodex agent is sitting on an integrator's marketing site, a "Request a Design Consultation" page, or an RFP-response landing page where you want a name and email before the agent dives into a discovery conversation.
Lead capture forms gate the chat behind a short form: name, email, phone, company, plus any custom fields you want (room count, project budget range, target standards, manufacturer preferences). Capture the lead, push it to Google Sheets, and let the agent personalize the conversation from there.

1. Open Collection Form
Go to the agent's Build page and click Collection Form under the Collect Leads section.
2. Connect Google Sheets (Optional)
The first screen asks if you want to push leads to a Google Sheet. If yes, connect your Google account and name the sheet (e.g. RFP Inquiries 2026).
3. Pick Form Fields
Choose from the presets:
- First Name
- Last Name
- Phone number
- Company name
Click a field to add it to the form.
4. Add Custom Fields
Click Create custom field to add text fields for anything else you need: project type, room count, target standards (Avixa, ISE, etc.), preferred manufacturers, target completion date.
5. Mark Fields Required or Optional
Use the toggle next to each field. Less is more here. Required fields kill completion rates.
6. Write the Form Message
Customize the message shown above the form to explain why you're collecting info. Example:
Tell us about your project and we'll route you to the right design engineer.
7. Save
Click Save to enable the form. Test it in the chat preview.

When you connect Google Sheets during setup:
- A new spreadsheet is created with your chosen name.
- Each form submission adds a new row.
- Column headers match your selected fields.

Enable / Disable#
Toggle the switch next to your form on the Build page to turn it on or off without deleting it.
Delete the Form#
Click the trash icon next to the form to remove it entirely.
View Submissions#
If the form is connected to Google Sheets, click the Sheets icon to open the spreadsheet and view every submission.
Collected info is available to the agent during the conversation. The agent can address the lead by name, reference their company, and tailor responses (different answers for a higher-ed AV manager versus a corporate IT director, for example).
Keep Forms Short#
Every extra field cuts completion rates. Only ask for what you actually need to route or qualify the lead.
Make Most Fields Optional#
Required fields create friction. Consider making phone and company optional unless they're critical to qualification.
Explain Why#
Use the form message to tell the user what you're going to do with their info:
Tell us about your project. A design engineer will follow up within one business day.
Form Not Appearing#
- Verify the form is enabled (toggle is on).
- Confirm at least one field is selected.
- Clear browser cache and test in incognito.
Leads Not Showing Up in Google Sheets#
- Confirm Google Sheets was connected during setup.
- Click the Sheets icon to verify the spreadsheet exists.
- Recreate the form with the Google Sheets connection if needed.
